We would rather know your family than your chart
Willow Creek opened in 2007 with two physicians in a converted house on Greenwood. We now have six providers and about 4,800 patients, but we hold panel sizes deliberately so appointments do not shrink to ten minutes. Many of the babies we delivered care for in 2008 are now bringing in their own kids' forms.

FAQ
Patient questions
Do you take new patients of all ages?
Yes, newborns through seniors. Call the front desk or use the form and we will verify your insurance and schedule a new patient visit, usually within two to three weeks. Newborns and urgent needs are worked in sooner.
What if my child gets sick over the weekend?
We hold a Saturday morning sick clinic from 9am to noon. Call at 8:30 to get on the list. For anything that cannot wait, including breathing difficulty or a high fever in an infant under three months, go to the emergency department.
When are sports physicals offered?
Year round by appointment, and we run walk in clinics on Saturdays in July and August ahead of fall sports. Bring the OSAA form and we will complete and sign it at the visit for 55 dollars.
How do prescription refills work?
Ask your pharmacy to send the request electronically and allow two business days. Medications requiring monitoring, such as blood pressure or thyroid, need a visit within the last twelve months before we can refill them.
