Booking Form Generator

Build a booking request form for a salon, restaurant, clinic, studio or any service business, and copy the HTML to paste into your site. Pick the fields you need, list your services, set the button text and color, and the generator writes labelled, accessible markup with the right input types for dates, times and numbers. Starter styles are included and optional.

What you get

How it works

  1. Choose your business type. It fills in a heading, a suggested list of services and a fitting label for the number-of-people field.
  2. Tick the fields to include: phone, service, date, time, number of people and notes. Name and email are always there so you can reply.
  3. Edit the services list (one per line), set the button text and color, and decide whether to include starter CSS.
  4. Copy the code and paste it into your page. Replace YOUR-FORM-URL with the address of whatever receives your form submissions.

What a good booking form asks for

The purpose of a booking request is to give you enough to say yes, not to replace the conversation. For most service businesses that means a name, a way to reach the person, the service, a preferred date and time and anything unusual. Restaurants add the size of the party; clinics and consultants add a short reason for the visit in the notes.

Everything else is a cost. Each additional required field gives a visitor another reason to leave, so save questions like address, birthday or how they heard about you for the confirmation step. If you must ask, make the field optional.

After the form is on the page

Test the whole path before announcing it: fill in the form on a phone, send it, and check that the message arrives where you expect and that you can read every field. Many forms look fine and silently send nothing because the action address is missing.

Plan the reply as well. A person who has just sent a request wants to know it was received and when to expect an answer. A short thank-you page or a confirmation email, with your opening hours and a number for changes, prevents double bookings and no-shows more than any extra form field will.

Tips

Frequently asked questions

Does this form take bookings by itself?

No. It generates the form's HTML and styling. To receive the entries you connect it to a form handler by putting that service's address in the action attribute, or by using the form tools built into your website platform.

Is the form accessible?

Each input has a visible label tied to it with matching for and id attributes, required fields are marked with the required attribute, the right input types are used, and autocomplete hints help browsers fill in names, emails and phone numbers. Colors are chosen so the button text stays readable.

Can I add or remove fields?

Use the checkboxes to include or leave out phone, service, date, time, number of people and notes. For anything else, copy the code and add another block in the same pattern: a div with a label and an input.

How are the services listed?

Each line in the services box becomes one choice in a drop-down menu. Leave the box empty to use the suggested list for the business type. Blank lines are ignored and special characters are escaped.

Does it work with my website builder?

Any platform that lets you add custom HTML or an embed block will take the code. Paste it in, then connect submissions to your email or a form service.
